If you would like to get involved please call Hanna at (320) 259-0393 or hanna@cmhp.net or attend the monthly Continuum of Care meeting. These meetings are the first Tuesday of the month from 1 p.m. - 3 p.m. in Little Falls. If you would like to have a presentation on the plan, please contact Hanna.
